Confidential Reporting Form

The purpose of this reporting tool is for students, staff and community members of Winona Area Public Schools to report situations and incidents involving, but not limited to, the following:

  • Harassment or discrimination prohibited by District Policy 413
  • Sex-based harassment or discrimination prohibited by District Policy 522
  • Disability-based harassment or discrimination prohibited by District Policy 521
  • bullying
  • suspicious behavior
  • sale or use of drugs
  • weapons
  • domestic violence or dating violence
  • concerns regarding a student or staff member
  • crimes against students, staff or the school district
  • any other non-academic issue that does not require immediate assistance, consistent with District Policy 103

This reporting tool is not monitored 24/7. For an immediate response to an emergency situation, please call 911 or use one of the following resources:

If your concern is about bullying, please report it to the building report taker pursuant to Policy 514. If your concern is about protected class harassment, other than sex-based harassment, please report it to the Human Rights Officer pursuant to Policy 413. If your concern is about sex-based harassment, please report it to the Title IX Coordinator pursuant to Policy 522. If your concern is about disability discrimination or harassment, please report it to the Section 504 Coordinator pursuant to District Policy 521.

Otherwise, before submitting your concern, first please consider whether your concern could be addressed by a WAPS teacher, counselor, the principal, or another administrator who you trust. If so, please contact
that person directly. Concerns regarding academic issues should be addressed with a teacher or school counselor.

Efforts will be placed on confidentiality, and all concerns will be evaluated and determined if an  investigation is necessary. Please note, district administration follow Policies 414 Mandated Reporting of
Child Neglect or Physical or Sexual Abuse and 415 Mandated Reporting of Maltreatment of Vulnerable Adults.

Reports can also be made directly to Winona Area Public Schools staff members through less formal forms of communication, including letters, emails, phone calls, voice mails or in-person conversations. Reports are classified and maintained in accordance with applicable law, including, but not limited to, the Minnesota Government Data Practices Act and the Family Educational Rights and Privacy Act, and will not be disclosed as permitted by applicable law.

The employee who receives this forms is typically the human resources director or designee. The complaint form will be submitted to the appropriate school district personnel pursuant to applicable policy. If If the complaint is about the Human Resources Director, the form will be reported to the Superintendent.
